Waiver Forms
Liability release forms (Waivers) must be signed by all game participants at the playing field. In addition, minority age participants must also have their parent or guardian sign the waiver. Parents are not required to sign in person, but the participant must confirm that the signature on the waiver is that of their parent.
Party organizers can download forms, or email the link to parents in their group. Please allow at least a week to gather completed forms so your games are not delayed.
